Social Workers in General Practice Pilot Program
CHN, ACT’s PHN funded the Social Workers in General Practice (SWiGP) pilot program to assist patients’ overall wellbeing and engagement with medical care. The program funded the employment of Social Workers as members of a multidisciplinary primary health care team across 4 general practices in the Australian Capital Territory (ACT).
Qualitative investigation of dementia specific ACT/SNSW HealthPathways
General practitioners and nursing and allied health professionals were invited by CHN and COORDINARE staff to participate in a discussion group to explore their experiences in dementia care in the ACT/SNSW and HealthPathways’ role in supporting them in their work.
Afterhours Home‐based Palliative Care Community Pharmacy
CHN, ACT’s PHN funded two pharmacies, Capital Chemist Wanniassa and Capital Chemist Charnwood to deliver medications in the after‐hours period for home‐based palliative care patients residing in the ACT.
In 2020, CHN commissioned the University of Canberra, Health Research Institute to evaluate the program. This report describes the key features of the program and provides an overview of evaluation findings including areas of strength, improvement and recommendations.
Pharmacists in Residential Aged Care Facilities (PiRACF) Study
Capital Health Network, through the Australian Government’s PHN Program, commissioned the University of Canberra to undertake this activity. Qualified pharmacists were integrated into residential aged care facilities (RACF) to improve medicine management quality and safety.
