About HealthPathways

HealthPathways is a dynamic collaboration between Capital Health Network (CHN), ACT Health, COORDINARE (SE NSW Primary Health Network), Southern NSW Local Health District and Streamliners NZ. The HealthPathways Team consists of program members based in ACT and SNSW and General Practitioner Clinical Editors.

HealthPathways is an online resource for information dissemination that supports clinical best practice to enable integration and system change. It offers clinicians locally agreed clinical and referral information to make the right decisions, together with patients, at the point of care. It is designed to map both public and private health and community services to support clinicians in guiding their patients through the health system.

About the role

The HealthPathways Program Coordinator works within the Quality Improvement Team at CHN to collaboratively deliver a range of program coordination to support the operational aspects of the HealthPathways program implementation. The role will focus to improve awareness and support broader adoption of HealthPathways in the local clinical community, to enable seamless and appropriate transitions in health care.

The role will also support the development and maintenance of HealthPathways by facilitating efficient collaboration between CHN’s Clinical Editing Team and Streamliners technical writing team, to produce standardised and most up-to-date clinical management information (pathways).

You will use high-level administrative skills to undertake a variety of coordination and operational duties to support the goals and objectives of the HealthPathways program. Your sound understanding of the ACT and national health system priorities will be essential to effectively navigate complex stakeholder engagement activities with medical specialists, General Practitioners, Nurses and other health service providers. Your ability to show initiative and be proactive will enable you to make a tangible positive impact in the patient journey and ultimately the health outcomes of the ACT community.

As part of working at an innovative organisation, you will also contribute to process improvement in the HealthPathways program representation, training provision and service development, as well as broader CHN projects and initiatives.

Work location

This position is based in our Deakin office. We support flexibility and a hybrid work model at CHN and are happy to work with the successful candidate to ensure balance between work and life.
