About the role

Project Coordinator Older Persons Health plays a pivotal role in supporting CHN’s aged care funding activities and programs, including General Practice in Aged Care Incentive (GPACI) and MyMedicare-related activities. This newly created role is also responsible for contract and stakeholder management for the Psychological Services in Residential Aged Care Homes (RACHs), requiring close collaboration with our network within the ACT primary health care community, including RACHs and CHN contracted service providers.

The successful candidate will interact regularly with internal teams to deliver cross functional collaborative projects contributing to high quality, sustainable models of care for older persons living in the Canberra region.


About the team

The Older Persons Health Planning overseas the development, implementation and management of activities and projects as part of an ongoing analysis of the needs of older persons in the ACT, contributing to delivery of better health outcomes. The Older Persons Health Planning team sits within a broader Health System Integration Business Unit, which is responsible for coordinating projects and program areas centered around improvement and integration of the health system to actively respond to the needs of the ACT community.

To effectively support the goals and objectives of the Older Persons Health Planning team, you will need to bring your adaptability, self-drive and creative confidence to undertake a variety of coordination and operational duties. A versatile Project Coordinator who combines technical project management expertise with a sensible approach to internal and external stakeholder relationships to ensure both effective collaboration and project success, will be a great fit for the role. Your ability to use tools such as Salesforce or Microsoft Office Suite to report on project deliverables, coupled with the added understanding of ACT’s primary health care sector will see you thrive in improving the health care services for older Australians.

Work location

This position is initially based in our Deakin office and then in Belconnen from late 2024/early 2025. We support flexibility and a hybrid work model at CHN and are happy to work with the successful candidate to ensure balance between work and life.

Position Salary: 

$47.33 per hour (approximately $93,832 per annum) plus superannuation
