What is HealthPathways?

• The ACT and SNSW HealthPathways Program is a free online platform for primary health care professionals that provides condition-based assessment, management, and referral information. The program is a unique cross-border partnership involving CHN (ACT PHN), ACT Health, Southern NSW Local Health District (SNSWLHD) and COORDINARE (South-Eastern NSW PHN). 
• The portal is not designed to be used by patients or the general community. However, there will be information available on the portal that can be provided to patients during a consultation. Watch this short introduction video to learn more about ACT and Southern NSW HealthPathways. 

How to access the HealthPathways site 

• All health professionals can register for access to the ACT & SNSW HealthPathways Platform for free.  

How to register   

ACT & SNSW HealthPathways users can now register for a personalised account to access the site. This means that users can create their own username and password. 

We are switching to personalised logins due to security requirements and to enable users to individualise their usage of HealthPathways. There are also upcoming new features that will only be available to users who register for a personalised account.
