Position Description
An opportunity exists to work within The Junction’s multidisciplinary team to provide holistic support and primary health care services to young people aged 12 to 25, with a particular focus on vulnerable or at-risk young people.
The practice offers generous scheduling to enable GP’s to spend additional time with complex clients and coordinate care requirements within a multidisciplinary context.
Clinical care, supervision and record keeping
- Provide primary health care assessment, diagnosis, treatment, support, education, referral and advocacy to young people and their children, within scope of knowledge, training and experience.
- Order diagnostic tests as clinically indicated, check and inform patients of results and enter appropriate reminders and recalls as per The Junction’s policy
Refer patients appropriately to other providers - Document all care provided along with all referrals and reasons for visits to align with medical documentation requirements and the Junction’s data collection requirements.
- Liaise with general practices, specialist services and allied health services across the ACT region
- Provide clinical support and guidance to The Junction treating team on a continuing basis
- Undertake data entry to ensure Anglicare reports to ACT Health on service delivery
- Maintain vocational registration through continued participation in Quality Assurance and Continuing Professional Development
- Have a working knowledge of legislation and standards of General Practice
- Actively participate in all supervision and performance management processes for the service
- Be actively involved in training, professional development and meetings, as required
- Maintain professional standards in relation to service delivery, confidentiality, maintenance of health care records and privacy – per The Junction Youth Health Service and Anglicare policies and procedures
- Work collaboratively with other team members to achieve service delivery excellence and actively participate in quality improvement activities to meet service/accreditation standards
- Provide prompt and courteous service to all young people and their families
- Promote a sense of belonging through engaging in meaningful, respectful and reciprocal interactions
- Acknowledge families as a valuable resource and develop and maintain positive relationships with them based on mutual respect and open communication.
Qualifications:
- Vocational Registration as a GP, with continued participation in Quality Assurance and Continuing Professional Development
- Clinical Review experience (desirable)
- A working knowledge of legislation and standards of General Practice
Key skills and experience:
- Demonstrated organisational and planning skills
- High-level verbal and written communication skills
- Experience working in a multidisciplinary team
- Previous experience in General Practice
- Minimum 2yrs experience working as a General Practitioner (desirable)
- Experience providing person centred care
Key knowledge areas:
- Knowledge of the needs of vulnerable young people and their families (desirable)
- Practical understanding and knowledge of child and adolescent development and the impact on development of relational trauma and abuse (desirable)
- Knowledge of ACT youth and health services networks (desirable)
Other Requirements:
- First Aid Certificate and CPR
- Driver’s License
- Current registration with AHPRA
- COVID Vaccination Status
Practice Description
The Junction Youth Health Service is funded through an ACT Government grant program to provide primary health care and support services to young people aged 12 to 25, along with their dependent children. The service is bulk-billed so there is no cost to patients who have a Medicare card. Special consideration to non-Medicare patients will be determined on a case-by-case basis.
We have a particular focus on young people who are homeless or otherwise experience (or are at risk of) disadvantage.
Services include:
- Medical assessment and review
- Nursing assessment and review
- Smoking and vaping cessation support
- Vaccinations
- Case management
- Youth work support
- Support groups
The Junction delivers services via its clinic at Scotts Crossing in Canberra City and also provides outreach services to young people at youth support services and out of home care locations via a specially equipped health van.
The Junction team consisting of GP’s, Mental Health GP, Practice Nurse and Youth Workers works collaboratively to support young people to solve a range of medical, psychosocial and practical issues so they can improve their health and participate in a range of activities in their communities.
