Capital Health Network, the ACT’s PHN is seeking to commission general practices in the ACT interested in participating in a pilot that will trial a collaborative intervention involving primary care, community-based care and the public health system to provide additional, early community-based support to patients at risk of presentation to the Emergency Department or admission to hospital to reduce their preventable risk of deterioration.
CHN is inviting applications from interested general practices to:
- Work with Canberra Health Services (CHS) to select patients for the pilot against a set of criteria.
- Agree to proactively manage these patients using a case management approach including participation in case conferencing and liaison with the CHS team as necessary.
- Access CHS specialist advice via telephone or virtual meeting to support management of their patient in the community.
- Provide appropriate access, care and referrals as necessary.
- Participate in specialist shared care programs as appropriate.
- Agree to provide free at point-of-care appointments for pilot patients in accordance with their care plan via their preferred funding mechanism and within the contraints of the Commonwealth Health Insurance Act 1973.

The EOI process commences Thursday 21 September 2023.
The closing date and time for this EOI is 5pm Friday 5 April 2024 or earlier if the quota of practices is filled before then. Information on any change to closing dates will be posted as soon as it is available.
There will be a phased approach to practice onboarding; delivered in 3 x 3-monthly Tranches. Five practices will be onboarded in a staggered approach each Tranche. Tranche 1 will commence in October 2023, Tranche 2 in January 2024 and Tranche 3 in April 2024.
